Have you sent Mountain Friends Camp your: 

-Registration form, electronic waiver (via jotform)

-Camper fees, with campership application if needed

–Camper Health Profile and Waiver (you complete via Jotform)

-Camper health insurance card/information (copy and attach to medical history form)

–MFC healthcare provider form (doctor/healthcare provider completes)

–MFC Immunization Waiver (if needed, for those without standard immunization history)

–MFC Pre-Arrival Health Screening and COVID-19 Test Results (complete 24 hours before arrival, with COVID-19 test results. Test earlier if you have any symptoms or exposure)
